Medical Records Request

You have the right to request and receive an electronic or paper copy of your medical records and other health information that we maintain about you. Requests must be submitted in writing and may require verification of your identity before records are released.

Please allow up to 15 business days for processing your request. In certain circumstances, an extension may be permitted as allowed by law.

A reasonable, cost-based fee may apply for copying, mailing, and other expenses associated with fulfilling your request not to exceed $25.
